Employment Application Checklist

All applications must include the following information so we can assess your suitability:

• Your current CV / Resume – including details of employment history; qualifications; registrations / licences / professional affiliations; contact details of at least 2 recent professional referees
• A covering letter – maximum of one page; informing the panel your reasons for applying for the position; detailing why your skills, experience and personal qualities make you the best person for the role; and of your availability to participate in rotating rosters &/or specific shifts.

Everyone who joins Western Private Hospital –

Must provide a current and acceptableNational Police Check (obtained no more than 6 months prior to commencement of employment), a current Working With Children Check (employee type) and evidence of Australian Residency / Full Working Rights (no visa sponsorships available)

Must provide proof of immunisationagainst, or immunity to, infectious diseases as required per the Health Services Act 1988 (Vic) and be vaccinated against COVID-19 and Influenza  – NOTE: depending on the role, healthcare workers have different immunisation requirements categorised into groups according to likelihood of their exposure to potentially infectious agents:- Risk Category A (direct contact with blood or body substances) OR Risk Category B (indirect contact with blood or body substances) OR Risk Category C (minimal patient contact)
