Access to a Patient Record

To request access to medical records for ongoing care

Please send your request to Health Information Services by:

  • Email – medrecords@westernprivate.com.au
  • Fax – (03) 9318 3590

Please ensure when sending your request;

  • via email, please use your professional health organisation email address.
  • via fax, please use your organisation’s letterhead.
  • Include patient’s full name and date of birth and specify what information you require.

Your GP or specialist is able to request information from your medical record for ongoing care by following the above process at no cost.

Patient information required by yourself for any other purpose will need to complete the Request to Access a Patient Record process outlined below. This will incur a fee and can take up to 45 days to process.

To request access to medical records for claims, clinical trial or study, personal use or other purposes

To request any information from your medical record, please complete the following process:

  • Download the Request to Access a Patient Record form.
  • Complete the form and save.
  • Attach the completed form with a copy of your photo identification and return it via

Submit your formal request to Health Information Services on medrecords@westernprivate.com.au

Please ensure that

  • Your request is submitted on your organisation official letterhead
  • Include the patient full name and date of birth and specify the information you are after.
  • Include patient signed consent for release of information

In accordance with the Health Records Act 2001. Western Private Hospital requires payment of fees for access to patient records and have up to 45 days to respond to your request. Fees are outlined on the Request to Access a Patient Record form. You will be invoiced for this service and payment is required prior to the release of information.
